Forks are placed to the left of the plate, the only exception is the fish fork, which is placed to the right.

Knives are placed to the inside, closest to the plate on the right side with the cutting side facing towards the plate. The sharper of the two, usually the steak knife, is on the very inside.

Spoons are placed to the right of the spoons. The soup spoon is placed to the outside of the teaspoons.

The best and easiest way to remember which to use is to begin at the outside and work your way in.

Water and wine glasses are placed to the upper right of the plate.

The bread and butter plate is to the left of the wine and water glasses.

The napkin has two common locations--folded and placed in the center of the dinner plate or to the left of the plate, usually under the forks.
